
Transitioning From School To Day Activities
This can be a daunting process but we aim to make this as easy as possible for both the individual and their families.
We have been completing transitions for school leavers for a number of years and work closely with the transition coordinators from quite a few schools including:
- Pītau-Allenvale School
- Ferndale Te Ahu
- Te Kura Tuarua O Horomaka- Hillmorton High School
- Te Pukenga- Cashmere Conductive Education
- Papanui High School
- Mairehau High School
- Rangiora High School
- Ashburton College
A personal transition plan is put in place for each young individual to ease them through the transition process.
The Process of Transitioning
What a transition plan will look like …
Initial contact is made from school, a parent(s) or EGL connector, which may include a visit to our Centre(s).
Term Three
- Meet with relevant parties at respective school for transition meeting.
- Manager and vocational support person to visit school to meet client in the school environment.
- Dates set for transition visit to respective Centre.
Term Four
- Transition visits (usually a half day) will commence with support from respective school – 2 to 3 visits.
- Student continues with the half day visits as set but with no school staff support.
- Students will have one or two whole day transition visits.
NEW YEAR – Students will start full time.
USEFUL INFORMATION
Open Hours – Monday to Friday 9:30am – 3:00pm
(please note that individual centres finish at 2:30pm to allow for a full staff meeting)
Monday – Kaiapoi
Wednesday – Hoon Hay
Thursday – Bishopdale
Transport
Transport during the day is included as part of an individual’s programme. However, it is the family’s or residential provider’s responsibility to transport the young person to and from the Centre each day.
